Phenelzine Sulfate falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Soy interacts with.

Taking soy products containing high amounts of tyramine along with MAOIs can increase the risk of hypertensive crisis.
Fermented soy products such as tofu and soy sauce contain tyramine, a naturally occurring chemical that affects blood pressure regulation. The metabolism of tyramine is decreased by MAOIs. Consuming more than 6 mg of tyramine while taking an MAOI can increase the risk of hypertensive crisis. The amount of tyramine in fermented soy products is usually less than 0.6 mg per serving; however, there can be significant variation depending on the specific product used, storage conditions, and length of storage. Storing one brand of tofu for a week can increase tyramine content from 0.23 mg to 4.8 mg per serving. Advise patients taking MAOIs to avoid fermented soy products that contain high amounts of tyramine.

Here's the key thing with phenelzine (Nardil): it blocks an enzyme called monoamine oxidase, which is also the enzyme your body uses to break down tyramine, a natural compound in certain foods. While you're on phenelzine, tyramine can pile up and cause a sudden, dangerous spike in blood pressure (a hypertensive crisis), with symptoms like a pounding headache, racing heart, sweating, or chest pain.

Plain soy or fresh tofu is usually low in tyramine, but fermented soy products like soy sauce, miso, and aged or improperly stored tofu can climb high enough to be risky. Tyramine levels jump the longer these sit. To stay safe, avoid fermented and aged soy foods while taking phenelzine, and ask your pharmacist or doctor before adding any soy supplement.

Phenelzine is used to treat depression — specifically the types described as atypical, nonendogenous, or neurotic depression. This often includes people who have a mix of anxiety and depression, or who have phobic or hypochondriacal features (strong fears about illness or physical symptoms).

It is generally not the first antidepressant a doctor will try. It is best suited for patients who have already tried other antidepressants without enough relief. There is less evidence supporting its use in severe depression with classic biological features (called endogenous depression).
