Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Soy interacts with.

Soy might modestly induce CYP2C9 enzymes. However, this effect does not seem to be clinically significant.
In vitro research suggests that an unhydrolyzed soy extract might induce CYP2C9. However, the significance of this interaction is likely minimal. In healthy females taking a specific extract of soy (Genistein Soy Complex, Source Naturals), blood levels of losartan, a CYP2C9 substrate, were not significantly affected.

Here's the gist for soy and your phenytoin. Soy contains isoflavones, and some lab (test tube) research hints they might slightly speed up CYP2C9, one of the liver enzymes that helps clear phenytoin. In theory, speeding that enzyme up could lower phenytoin levels a bit, which could mean less seizure protection. But this is only a weak, theoretical signal. In a small human study, a soy product didn't meaningfully change levels of another CYP2C9 drug.

Phenytoin is a narrow-window medicine, so even small shifts can matter. Still, normal soy foods are very unlikely to cause trouble. If you want to start concentrated soy isoflavone supplements, just check with your pharmacist or doctor first.

About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
