Here is the interaction we found between these two, with the evidence behind it.
ANTIBIOTIC DRUGS
Why this applies to Methenamine Mandelate
Methenamine Mandelate falls under ANTIBIOTIC DRUGS — the group Soy interacts with.
Intestinal bacteria are responsible in part for converting soy isoflavones into their active forms. Antibiotics may decrease the amount of intestinal bacteria and decrease its ability to convert isoflavones.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for methenamine, which is a urinary antiseptic used to help prevent bladder infections. The interaction on file actually runs the other direction from what people usually worry about. It's not that the medicine becomes dangerous, it's that methenamine (acting like an antibiotic) could slightly lower how well your body activates soy isoflavones. Your gut bacteria help convert those soy compounds into their useful forms, and antimicrobials can trim back some of those bacteria.
So your methenamine should keep working fine, but you might get a touch less benefit from soy. Honestly, this is theoretical and based only on lab work, so I wouldn't lose sleep over it. Everyday soy foods are very unlikely to matter. If you take concentrated soy isoflavone supplements, just mention it to your pharmacist.

About Soy
What is Soy?
- Menopause symptoms (hot flashes)
- Heart and cholesterol health
- Bone health
- Plant-based protein source
- General nutrition
Soy is a nutritious bean that is a staple food and a popular source of plant protein and isoflavones. Eating soy foods as part of a balanced diet is generally considered safe for most people, and there is moderate evidence it may modestly help cholesterol and some menopause symptoms, though results vary. Talk with your pharmacist or doctor before using concentrated soy isoflavone supplements, especially if you have a history of hormone-sensitive conditions.
About Methimazole
What Is Methimazole Used For?
Brand names: Mandelamine, Methimazole
Methimazole tablets are used in adults and children with hyperthyroidism (an overactive thyroid) due to Graves' disease or toxic multinodular goiter when surgery or radioactive iodine is not the right option. They work to lower thyroid hormone levels and ease symptoms.
Methimazole tablets are also used to bring thyroid hormone levels down before a patient undergoes thyroidectomy (thyroid surgery) or radioactive iodine treatment, making those procedures safer.
