Soy and Metformin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIDIABETES DRUGS

Why this applies to Metformin

Metformin falls under ANTIDIABETES DRUGS — the group Soy interacts with.

Soy can lower blood glucose and have additive effects with antidiabetes drugs.
Clinical research shows that whole soy diets and soy-based meals reduce fasting glucose levels in diabetic and non-diabetic individuals. Also, individuals following a soy-based meal replacement plan seem to require lower doses of sulfonylureas and metformin to manage blood glucose levels when compared with individuals following a diet plan recommended by the American Diabetes Association.
Severity Moderate Likelihood Possible Evidence A

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: soy itself can gently nudge your blood sugar downward. Studies show that eating a soy-rich diet tends to lower fasting glucose, even in people without diabetes. So when you pair soy with metformin, which is already working to bring your numbers down, the two can add up. This isn't soy changing how your body breaks down the drug. It's more like two helpers pushing in the same direction.

For most people this is mild, and good-quality research supports the effect. The main thing to watch for is blood sugar dipping a little lower than usual (think shakiness, sweating, or feeling lightheaded), especially if you switch to a heavy soy-based eating plan or take concentrated isoflavone supplements. Keep an eye on your home readings, and chat with your doctor or pharmacist before making any changes.

From our pharmacist’s overview on the Metformin page

“Siberian cocklebur is the ingredient I most want anyone taking metformin (Glucophage) to know about. It has caused severe low blood sugar in people, documented in humans, and it should not be combined with any glucose-lowering medication.”

About Soy

What is Soy?

  • Menopause symptoms (hot flashes)
  • Heart and cholesterol health
  • Bone health
  • Plant-based protein source
  • General nutrition

Soy is a nutritious bean that is a staple food and a popular source of plant protein and isoflavones. Eating soy foods as part of a balanced diet is generally considered safe for most people, and there is moderate evidence it may modestly help cholesterol and some menopause symptoms, though results vary. Talk with your pharmacist or doctor before using concentrated soy isoflavone supplements, especially if you have a history of hormone-sensitive conditions.
