Belzutifan falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group St. John's Wort interacts with.

St. John's wort may increase the metabolism and reduce the levels of CYP2C19 substrates.
Preliminary clinical research in healthy males shows that taking St. John's wort for 14 days induces CYP2C19 and increases metabolism of mephenytoin (Mesantoin). In patients with wild-type 2C19 (2C19*1/*1) metabolism was almost 4-fold greater in subjects who received St. John's wort compared to placebo. In contrast, patients with 2C19*2/*2 and *2/*3 genotypes did not demonstrate a similar increase in metabolism. Theoretically, St. John's wort might increase metabolism of other CYP2C19 substrates.

Here's the concern with pairing St. John's wort and Welireg (belzutifan). Belzutifan is cleared from your body partly by a liver enzyme called CYP2C19. St. John's wort tends to speed up that enzyme, which means your body may break belzutifan down faster and leave less of it in your bloodstream.

For a cancer medicine like this, weaker drug levels are the worry, because you may not get the full benefit you're counting on to control your tumors. Belzutifan is not a prodrug, so the herb works against it rather than boosting it. The evidence here is fairly preliminary, but with a serious condition like VHL, it's not worth the gamble. Please don't add St. John's wort without first checking with your oncology team or pharmacist.

About Belzutifan

What Is Belzutifan Used For?

Brand name: Welireg

Welireg (belzutifan) is approved for several serious conditions. In adults with von Hippel-Lindau (VHL) disease, it treats associated kidney cancer (renal cell carcinoma), brain and spine tumors called CNS hemangioblastomas, and pancreatic neuroendocrine tumors — when surgery is not immediately needed.

Welireg also treats adults with clear cell renal cell carcinoma (ccRCC): as an adjuvant treatment (after surgery to remove the kidney) in combination with pembrolizumab or pembrolizumab and berahyaluronidase alfa-pmph to reduce the risk of recurrence, and as a single agent for advanced ccRCC after certain prior therapies have been tried. Additionally, Welireg treats adults and patients aged 12 and older with locally advanced, unresectable, or metastatic pheochromocytoma or paraganglioma (PPGL) — rare tumors of the adrenal glands or nearby nerve tissue.
