Belzutifan falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Baikal Skullcap interacts with.

Theoretically, Baikal skullcap might increase levels of drugs metabolized by CYP2C19 enzymes.
In vitro evidence suggest that wogonin, a constituent of Baikal skullcap, modestly inhibits the activity of CYP2C19 enzymes. This effect has not been reported in humans.

Here's the gist: your kidney cancer medicine Belzutifan (Welireg) is cleared from your body partly by a liver enzyme called CYP2C19. Baikal skullcap contains a compound named wogonin that, in lab dish studies, mildly slowed that same enzyme down. If that were to happen in a real person, belzutifan could build up a bit higher than expected, which might mean stronger side effects like low red blood cell counts (anemia), low oxygen levels in the blood, fatigue, dizziness, or headache.

I want to be honest about the evidence here: this is only seen in test tube studies, not in actual patients, so it may not matter at all. Still, belzutifan is a serious cancer drug, so please check with your oncologist or pharmacist before adding this herb.

About Belzutifan

What Is Belzutifan Used For?

Brand name: Welireg

Welireg (belzutifan) is approved for several serious conditions. In adults with von Hippel-Lindau (VHL) disease, it treats associated kidney cancer (renal cell carcinoma), brain and spine tumors called CNS hemangioblastomas, and pancreatic neuroendocrine tumors — when surgery is not immediately needed.

Welireg also treats adults with clear cell renal cell carcinoma (ccRCC): as an adjuvant treatment (after surgery to remove the kidney) in combination with pembrolizumab or pembrolizumab and berahyaluronidase alfa-pmph to reduce the risk of recurrence, and as a single agent for advanced ccRCC after certain prior therapies have been tried. Additionally, Welireg treats adults and patients aged 12 and older with locally advanced, unresectable, or metastatic pheochromocytoma or paraganglioma (PPGL) — rare tumors of the adrenal glands or nearby nerve tissue.
