Here's the concern with pairing St. John's wort and boceprevir (Victrelis). This herb revs up a liver enzyme called CYP3A4, which is the same enzyme that breaks down boceprevir. When that enzyme works faster, your body clears boceprevir more quickly, so the levels in your blood drop.

Why does that matter? Boceprevir is a hepatitis C medicine that needs steady, adequate levels to keep the virus in check. If the herb pulls those levels down, the drug may not work as well, and you could end up with treatment failure or even help the virus become resistant. This is a major, well-documented interaction, so St. John's wort and boceprevir really shouldn't be combined. Please check with your pharmacist or doctor before adding any herbal product.

St. John's wort decreases the levels and clinical effects of P-glycoprotein substrates.
St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.

Here's the concern if you pair St. John's wort with boceprevir (Victrelis). St. John's wort revs up a transporter in your gut called P-glycoprotein, and it also speeds up liver enzymes that handle many drugs. The result is that less boceprevir gets absorbed and stays in your system, so blood levels drop.

That matters because boceprevir is fighting an active hepatitis C infection. If its levels fall, the virus may not be controlled well, and you risk treatment failure or even helping the virus become resistant. This is a major, well-documented interaction, so St. John's wort really shouldn't be combined with boceprevir. Please don't start or stop anything on your own. St. John's wort might decrease the levels and clinical effects of boceprevir.
Boceprevir increases the maximum concentration and concentration at 8 hours of the St. John's wort constituent, hypericin, by approximately 30%. However, St. John's wort does not significantly change the area under the concentration-time curve or maximum plasma concentration of boceprevir 800 mg three times daily in healthy adults.

Here's the gist for boceprevir (Victrelis), which you take with ribavirin and peginterferon to clear hepatitis C. St. John's wort is a strong activator of the body's drug-clearing systems, so on paper it could speed up how fast you break down boceprevir and lower its levels. If that happened, the worry would be a weaker punch against the virus, meaning the treatment might not work as well as it should.

That said, the actual study data here is reassuring. In healthy adults, St. John's wort did not meaningfully change boceprevir's blood levels, which is why this is rated a minor, possible interaction on lower-quality evidence. Still, since you're working hard to beat hep C, I wouldn't risk it. Please check with your pharmacist or doctor before pairing these.
