Ethinyl Estradiol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group St. John's Wort interacts with.
St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the key thing to know: ethinyl estradiol is the estrogen in many birth control pills, and your liver clears it using an enzyme called CYP3A4. St. John's wort speeds that enzyme up, so it pushes the estrogen out of your body faster. That means there's less of it working than you'd expect.
In real life, this can make the pill less reliable. People taking the two together have reported breakthrough bleeding or spotting, and there are documented cases of unintended pregnancy. This is a well-recognized, meaningful interaction, not just a theory. Please don't stop either one on your own. Talk with your pharmacist or doctor first so they can suggest a backup method or a different plan.

P-GLYCOPROTEIN SUBSTRATES

St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.

Here's the key thing to know if you're on ethinyl estradiol (the estrogen found in many birth control pills): St. John's wort speeds up how your body processes this hormone, so less of it stays in your system. With lower estrogen levels, the pill becomes less reliable. In real terms, that can mean breakthrough bleeding or spotting, and most importantly, a higher chance of an unintended pregnancy.
This isn't just a theory. Several studies have shown St. John's wort can reduce how well hormonal birth control works, which is why it's flagged as a major interaction. Ethinyl estradiol is an active hormone (not a prodrug), so faster clearance simply means weaker protection. Here's the bottom line for ethinyl estradiol, the estrogen found in many birth control pills and some hormone products. St. John's wort speeds up the liver enzymes that break this hormone down, so it can clear out of your body faster than it should. That means lower hormone levels and a weaker effect.
For birth control, this is the big worry. People have gotten pregnant or had breakthrough bleeding while taking St. John's wort with their pill. The herb's strongest effect is actually on a different enzyme (CYP3A4), but it nudges CYP1A2 too, and both help clear this estrogen.
