St. John's wort increases the metabolism and reduces the levels of CYP3A4 substrates.
St. John's wort induces CYP3A4 enzymes and increases metabolism of CYP3A4 substrates. Clinically significant interactions have been reported with St. John's wort products containing hyperforin 1 mg or more.

Here's the gist with St. John's wort and trazodone: your liver clears trazodone using an enzyme called CYP3A4, and St. John's wort revs that enzyme up. The result is that your body may break down trazodone faster, leaving less of it around. That can make it work less well, so your depression or sleep trouble may not be controlled as well as before.

There's also a second concern. Both St. John's wort and trazodone raise serotonin, so combining them could, in rare cases, push serotonin too high (think agitation, sweating, fast heartbeat, shaking). Either way, please don't change anything on your own. Chat with your pharmacist or doctor first so they can guide you safely.

Theoretically, St. John's wort might inhibit reuptake and increase levels of serotonin, resulting in additive effects with serotonergic drugs.
Concomitant use of serotonergic drugs with St. John's wort can lead to increased adverse effects and increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstriction disorders.

Here's the thing to keep in mind: both St. John's wort and trazodone push serotonin levels up in the brain. Trazodone already works by boosting serotonin to lift mood, and St. John's wort seems to do something similar. Stack them together and you can get too much serotonin activity, which is what we worry about.

Realistically that might show up as agitation, sweating, shakiness, a racing heartbeat, confusion, or in rare serious cases what we call serotonin syndrome. I'll be honest, this concern is mostly theoretical, based on how the two work rather than lots of real-world reports, so I don't want to scare you. Still, before combining them, please check with your pharmacist or doctor first.
