Here's the gist for these two together. Your body clears zolpidem mainly through a liver enzyme called CYP3A4. St. John's wort revs that enzyme up (it speeds it along), so your system breaks down zolpidem faster than usual. The likely result is that the sleep medicine becomes weaker, and you may find yourself lying awake again or waking through the night, basically feeling like your Ambien just isn't working anymore.

This is a real, well-documented effect with St. John's wort products, especially ones containing meaningful amounts of hyperforin. Please don't change either one on your own. A quick chat with your pharmacist or doctor can help you sort out a safer plan.

St. John's wort might decrease the levels and clinical effects of zolpidem.
In clinical research, concomitant use of zolpidem with St. John's wort decreased zolpidem plasma levels due to induction of its metabolism by cytochrome P450 3A4. Whether this interaction is clinically significant is not known.

Here's the gist for these two: your body clears zolpidem using a liver enzyme called CYP3A4, and St. John's wort tends to speed that enzyme up. When you take them together, your sleep aid can get broken down faster, so less of it sticks around. In a clinical study, blood levels of zolpidem actually dropped.

What that means in real life is that your Ambien may not work as well, and you could find yourself tossing and turning or waking up more than usual. Zolpidem is not a prodrug, so this is about weaker sleep help, not extra side effects. Researchers aren't sure how big a deal this is for everyone, so before mixing them, check with your pharmacist or doctor.

About Zolpidem

What Is Zolpidem Used For?

Brand names: Ambien CR, Intermezzo, Sublinox

Zolpidem is used short-term for insomnia. Most formulations — including Ambien, oral tablets, and Zolpidem Tartrate Capsules — are approved to help people who have trouble falling asleep at bedtime. Zolpidem Tartrate Capsules are specifically indicated for adults under 65.

The extended-release formulations (including Zolpidem Tartrate Extended-Release Tablets) help with both falling asleep and staying asleep through the night. Sublingual tablets (Edluar, Zolpidem Tartrate Sublingual Tablet, and Zolpidem tartrate sublingual) are taken as needed when you wake up in the middle of the night and cannot get back to sleep — but only if at least 4 hours of sleep time remain before you need to be up.
