Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Abemaciclib
Abemaciclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Syrian Rue interacts with.
Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear abemaciclib (Verzenio) out of your system. Lab studies suggest the harmala alkaloids in Syrian Rue can slow that same enzyme down. If that happens in people too, abemaciclib could build up to higher levels than intended, which may mean stronger side effects like diarrhea, low blood counts, fatigue, nausea, or, more rarely, liver or clotting problems.
The important caveat: this is theoretical, based only on test-tube research, not real patient cases. Still, abemaciclib is a serious cancer drug, so I wouldn't combine it with Syrian Rue without checking first. Please talk with your oncologist or pharmacist before using this supplement.

- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Abemaciclib page
“A daily pill for a common form of advanced breast cancer, abemaciclib (Verzenio) only helps when its blood level stays steady, so this list of 138 supplements deserves genuine care. Nearly all of them act on CYP3A4, the liver enzyme that clears abemaciclib, meaning a supplement can push its level up and intensify side effects, or drag…”

About Syrian Rue
What is Syrian Rue?
- Traditional remedy for digestive complaints
- Used as a sedative or for mood
- Antimicrobial and antiparasitic folk use
- Used as a dye and incense
- Recreational psychoactive use
Syrian Rue (Peganum harmala) is a traditional plant whose seeds contain potent compounds called harmala alkaloids, including MAO-inhibiting harmine and harmaline. There is little reliable human evidence that it safely treats any condition, and it carries real risks of toxicity, so it is not recommended for self-treatment. Talk to a pharmacist or doctor before considering it.
About Abemaciclib
What Is Abemaciclib Used For?
Brand name: Verzenio
Verzenio (abemaciclib) is used to treat adults with hormone receptor (HR)-positive, HER2-negative breast cancer. For early breast cancer at high risk of returning, Verzenio is taken alongside endocrine therapy (tamoxifen or an aromatase inhibitor) after surgery to help prevent recurrence.
For advanced or metastatic breast cancer, Verzenio can be used in three ways: combined with an aromatase inhibitor as a first hormonal treatment; combined with fulvestrant after the cancer has progressed on prior hormone therapy; or on its own after both hormone therapy and prior chemotherapy have stopped working.
