Here's the gist: pramlintide (Symlin) already works hard to keep your blood sugar from spiking after meals, and you pair it with mealtime insulin. Tamarind seed extract might nudge blood sugar a little lower too. Put them together and the worry is simply additive: your numbers could dip more than expected, raising the risk of low blood sugar (hypoglycemia), which can feel like shakiness, sweating, dizziness, confusion, or a racing heart.

I want to be honest about the evidence though. This is based on animal studies using concentrated tamarind seed extract, not the tamarind you cook with. Eating tamarind in food is very unlikely to cause a problem. If you're considering a tamarind supplement, check your blood sugar closely and talk with your pharmacist or doctor first, since your doses may need a tweak.

SymlinPen (pramlintide) is used as an add-on treatment for adults with type 1 or type 2 diabetes who already use mealtime insulin but still haven't reached their target blood sugar levels — even with optimal insulin therapy.

It is not a replacement for insulin. It works alongside insulin, given right before major meals, to reduce the spike in blood sugar that happens after eating.
