Here's the simple version: repaglinide already lowers your blood sugar by nudging your pancreas to release insulin. The thought is that concentrated tamarind seed extract might add a little extra blood-sugar-lowering on top of that. If both push in the same direction, your sugar could drop too low (hypoglycemia), which can show up as shakiness, sweating, a fast heartbeat, dizziness, or feeling confused.

Now, the honest part: this concern comes from animal studies only, not from people, so it's more of a theory than a proven problem. Enjoying tamarind as a food in cooking is very unlikely to matter. If you're thinking about a concentrated tamarind supplement, check with your pharmacist or doctor first and keep an eye on your blood sugar readings.
