Cholestyramine falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the simple version: cholestyramine (Questran) works by grabbing onto things in your gut so they get carried out instead of absorbed. Unfortunately, it doesn't just grab cholesterol and bile acids. It also binds to thyroid hormone, so the T4 and T3 in a thyroid extract can stick to the cholestyramine and never fully get into your body. The result is that you absorb less of the thyroid hormone you were hoping to get.

This is a well-documented effect, though it's usually considered minor and is easy to manage with timing. The common fix is to separate the two by several hours, often taking the thyroid product around 4 hours before or after the cholestyramine. Since over-the-counter thyroid extracts are not standardized to begin with, please loop in your doctor or pharmacist before relying on one alongside Questran.

  • Cholestyramine is used with diet changes (restriction of cholesterol and fat intake) to reduce the amount of cholesterol and certain fatty substances in your blood. Accumulation of cholesterol and fats along the walls of your arteries (a process known as atherosclerosis) decreases blood flow and, therefore, the oxygen supply to your heart, brain, and other parts of your body. Lowering your blood level of cholesterol and fats may help to prevent heart disease, angina (chest pain), strokes, and heart attacks.
