Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs in one direction: taking your hydroxyurea (Hydrea, Droxia, or Siklos) around the same time as a thyroid extract supplement may keep the thyroid hormone from absorbing well. Thyroid hormones are picky and absorb best on an empty stomach, so having another pill in your stomach at the same time can blunt how much thyroid you actually take in. This is rated minor, and it's really about the thyroid product, not your hydroxyurea, which works just fine regardless.

Hydroxyurea is its own active drug, not a prodrug, so the thyroid extract isn't expected to make it stronger or weaker. My honest advice: skip unregulated thyroid extracts (the hormone amounts are unpredictable) and let your doctor or pharmacist guide any real thyroid treatment.

What Is Hydroxyurea Used For?

Brand names: Droxia, Hydrea, Siklos, Xromi

Hydroxyurea is used for two main groups of conditions. Which one depends on the product.

  • Sickle cell anemia: Droxia (capsule), Siklos (tablet) and Xromi (oral solution) reduce the frequency of painful crises and the need for blood transfusions in people with recurrent moderate to severe painful crises.
  • Cancer: Hydrea and hydroxyurea capsules treat resistant chronic myeloid leukemia. They also treat locally advanced squamous cell carcinomas of the head and neck (excluding the lip), used together with chemoradiation.

Age ranges differ by product. Siklos is for ages 2 and older. Xromi is for ages 6 months and older.
