Tinospora Cordifolia + Fentanyl
Fentanyl brand names: Sublimaze, Duragesic, Fentora, Lazanda, Abstral, and others.
Tinospora Cordifolia has a moderate interaction with Fentanyl. Theoretically, Tinospora cordifolia might increase levels of drugs metabolized by CYP2D6.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
Why this applies to Fentanyl
Fentanyl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Tinospora Cordifolia interacts with.
In vitro research shows that Tinospora cordifolia extract inhibits CYP2D6 at high concentrations. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with Giloy and fentanyl. The concern on file is that compounds in Tinospora may block an enzyme called CYP2D6, which could let drugs that depend on it build up in your body. The catch is that fentanyl is mostly handled by a different enzyme (CYP3A4), not CYP2D6, so this particular interaction may not push fentanyl levels much at all.
That said, this is only lab-dish data at high concentrations, never confirmed in real people. Because fentanyl is such a powerful opioid, even small shifts matter, and over-sedation, slowed breathing, or extreme drowsiness can be dangerous. Please don't change anything on your own. Run this by your pharmacist or doctor before adding Giloy.

From our pharmacist’s overview on the Fentanyl page
“There is very little room for error with fentanyl (Duragesic, Sublimaze, Actiq, and other forms), a powerful opioid where a modest rise in blood level or an extra layer of sedation can become dangerous, so this interaction list deserves real respect. Most of the risk falls into two patterns.”
