Uzara and Benzthiazide

Here is the interaction we found between these two, with the evidence behind it.

Moderate

DIURETIC DRUGS

Why this applies to Benzthiazide

Benzthiazide falls under DIURETIC DRUGS — the group Uzara interacts with.

Theoretically, concomitant use of potassium-depleting diuretics and uzara can increase the risk of cardiac glycoside toxicity due to potassium depletion. Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the concern with pairing these two. Benzthiazide (Exna) is a thiazide-type water pill, and like most diuretics in this family, it can flush extra potassium out of your body. Uzara root naturally contains cardiac glycosides, the same kind of heart-active compounds found in certain heart drugs. When your potassium runs low, those glycosides can hit your heart harder, raising the chance of cardiac glycoside toxicity, which can show up as nausea, vision changes, dizziness, or an irregular or unusually slow heartbeat.

I want to be honest: this is a theoretical interaction based on how these substances work, not on strong human studies. Still, since both touch the heart, it's worth a quick chat with your pharmacist or doctor before combining them, especially if your potassium tends to run low.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Benzthiazide page

“Benzthiazide (Exna), a thiazide water pill prescribed for high blood pressure and fluid retention, comes with a supplement list I would treat with genuine respect. Most of what appears here either drops blood pressure further than intended or deepens the potassium and fluid loss the drug already causes, and a handful can strain the kid…”
