Vitamin D and Diltiazem, Enalapril

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Vitamin D interacts with whole categories of medication. Diltiazem, Enalapril falls into 2 of those categories, so it can interact with Vitamin D in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

DILTIAZEM (Cardizem, others)

1 of 2
Theoretically, hypercalcemia induced by high-dose vitamin D can reduce the therapeutic effects of diltiazem for arrhythmia.
High doses of vitamin D can cause hypercalcemia. Hypercalcemia can reduce the effectiveness of verapamil in atrial fibrillation. Theoretically this could also occur with diltiazem. Avoid vitamin D doses above the tolerable upper intake level (4000 IU daily for adults) and monitor serum calcium levels in people taking vitamin D and diltiazem concurrently.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the gist for your Teczem, which combines diltiazem (a calcium channel blocker) with enalapril (a blood pressure medicine). The concern isn't with normal vitamin D doses. It's only if you take very high amounts over time, which can push your blood calcium too high. Since diltiazem works partly by affecting calcium movement in heart cells, extra calcium in the blood could blunt how well it controls a fast or irregular heartbeat.

This is mostly theoretical and seen more with verapamil, a close cousin of diltiazem. A typical daily vitamin D supplement is very unlikely to cause trouble. Just stay at or below 4000 IU a day unless your doctor advised more, and check with your pharmacist if you're on high-dose D so they can keep an eye on your calcium.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Why this applies to Diltiazem, Enalapril

Diltiazem, Enalapril falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Vitamin D interacts with.

Vitamin D might induce CYP3A4 enzymes and reduce the bioavailability of CYP3A4 substrates.
There is some concern that vitamin D might induce CYP3A4. In vitro research suggests that vitamin D induces CYP3A4 transcription. Additionally, observational research has found that increased UV light exposure and serum vitamin D levels are associated with decreased serum levels of CYP3A4 substrates such as tacrolimus and sirolimus, while no association between UV light exposure or vitamin D levels and levels of mycophenolic acid, a non-CYP3A4 substrate, was found. A small, low-quality clinical study shows that taking vitamin D reduces levels of the CYP3A4 substrate atorvastatin and its active metabolites by up to 55%; however, the clinical effects of atorvastatin were not reduced. While researchers theorized that vitamin D might induce CYP3A4, this proposed mechanism was not specifically studied.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

This combination pill pairs diltiazem (a blood pressure and heart-rate medicine) with enalapril (an ACE inhibitor). The diltiazem half is processed by a liver enzyme called CYP3A4, and there is a theory that vitamin D may slightly speed that enzyme up. If that happened, your body could clear diltiazem a bit faster, which in theory might make it work less well. That could mean blood pressure or heart rate creeping back up.

Honestly though, this is a minor, theory-only concern based mostly on lab studies, and normal vitamin D doses are very unlikely to change how your medicine works. Enalapril isn't really affected here. Keep taking your vitamin D as directed, and just check in with your pharmacist or doctor if you have questions.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.

From our pharmacist’s overview on the Diltiazem, Enalapril page

“Anyone taking Teczem (diltiazem with enalapril) for blood pressure should know that most of the 271 supplements with an interaction on file hit the same targets as the drug itself. The bulk either add to its blood-pressure-lowering effect or change how the body handles diltiazem, which is processed by an enzyme called CYP3A4.”

About Vitamin D

What is Vitamin D?

Vitamin D
  • Bone health and osteoporosis
  • Correcting vitamin D deficiency
  • Immune system support
  • Muscle strength in older adults
  • General supplementation when sun exposure is low

Vitamin D is a fat-soluble vitamin that helps your body absorb calcium and is important for healthy bones, muscles, and immune function. Many people, especially those with low sun exposure, benefit from supplementation, but more is not always better and very high doses can be harmful. Ask your pharmacist or doctor whether a blood test and supplement make sense for you.
