Chlorthalidone falls under THIAZIDE DIURETICS — the group Vitamin D interacts with.

Theoretically, taking thiazide diuretics and high-dose vitamin D can increase the risk of hypercalcemia.
Thiazide diuretics decrease urinary calcium excretion, which could lead to hypercalcemia if vitamin D supplements are taken concurrently. This has been reported in people being treated with vitamin D for hypoparathyroidism, and also in elderly people with normal parathyroid function who were taking a thiazide, vitamin D, and calcium-containing antacids daily.

Here's the simple version: chlorthalidone, your water pill, nudges your kidneys to hold onto a little more calcium instead of flushing it out in your urine. On its own that's usually no big deal. But pair it with high-dose vitamin D, which helps your gut pull more calcium out of food, and the two can stack up. In some people that has led to too much calcium in the blood (hypercalcemia), with symptoms like nausea, constipation, feeling thirsty, frequent urination, weakness, or confusion.

I want to be honest about the strength here: this is mostly theoretical and based on scattered case reports, often in folks also taking calcium antacids. A normal daily vitamin D dose is unlikely to cause trouble. Still, if you take big doses or extra calcium, let your pharmacist or doctor know so they can check a calcium level. Please don't stop either one on your own.

What Is Chlorthalidone Used For?

Brand names: Hygroton, Thalitone

Chlorthalidone is used to treat high blood pressure (hypertension) in adults. Lowering blood pressure reduces the risk of serious cardiovascular events, especially strokes and heart attacks. It can be used alone or combined with other blood pressure medicines.

Chlorthalidone is also used as an add-on treatment for edema (fluid buildup) caused by heart failure, liver cirrhosis, and kidney conditions including nephrotic syndrome, acute glomerulonephritis, and chronic renal failure. Thalitone is additionally indicated for edema associated with heart failure, liver cirrhosis, and renal disease including nephrotic syndrome.
