Zinc and Eravacycline

Here is the interaction we found between these two, with the evidence behind it.

Moderate

TETRACYCLINE ANTIBIOTICS

Why this applies to Eravacycline

Eravacycline falls under TETRACYCLINE ANTIBIOTICS — the group Zinc interacts with.

Zinc might reduce levels of tetracycline antibiotics.
Tetracyclines form complexes with zinc in the gastrointestinal tract, which can reduce absorption of both the tetracycline and zinc when taken at the same time. Taking zinc sulfate 200 mg with tetracycline reduces absorption of the antibiotic by 30% to 40%. Demeclocycline and minocycline cause a similar interaction. However, doxycycline does not seem to interact significantly with zinc. Advise patients to take tetracyclines at least 2 hours before, or 4-6 hours after, zinc supplements to avoid any interactions.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the good news with Xerava (eravacycline): it's given as an IV infusion straight into your vein, not as a pill you swallow. The classic zinc and tetracycline problem happens in the gut, where zinc binds to the antibiotic and keeps it from being absorbed. Since eravacycline skips your stomach entirely, that binding issue really shouldn't apply here.

So in practice, your zinc supplement is very unlikely to weaken eravacycline or affect how well your infection clears. This file note was written about oral tetracyclines like the older capsules, which do need that 2-hour spacing. That said, I'd still mention all your supplements to the team treating you, since they have the full picture of your care.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

About Eravacycline Injection

What Is Eravacycline Injection Used For?

Brand name: Xerava

Xerava (eravacycline) is used to treat complicated intra-abdominal infections (serious infections inside the abdomen, such as those following bowel problems or surgery) in adults 18 and older. It targets a range of bacterial types, including E. coli, Klebsiella pneumoniae, Enterococcus species, Staphylococcus aureus, Bacteroides species, and several others identified by lab testing.

Xerava is not approved for complicated urinary tract infections (cUTI). It should only be used when a bacterial infection is proven or strongly suspected — using antibiotics unnecessarily contributes to antibiotic resistance.
