AMILORIDE (Midamor)

Amiloride can modestly reduce zinc excretion and increase zinc levels.
Clinical research shows that amiloride can reduce urinary zinc excretion, especially at doses of 10 mg per day or more. This zinc-sparing effect can help to counteract zinc losses caused by thiazide diuretics, but it is unlikely to cause zinc toxicity at usual amiloride doses. The other potassium-sparing diuretics, spironolactone (Aldactone) and triamterene (Dyrenium), do not seem to have a zinc-sparing effect.

Here's the interesting thing about this combo pill. The hydrochlorothiazide part tends to make your kidneys flush out a little extra zinc, but the amiloride part works in the opposite direction, holding onto zinc and slowing how much you lose in your urine. So in this particular medication, the two ingredients roughly balance each other out when it comes to zinc.

The bottom line is that this is a minor interaction. Amiloride may modestly raise your zinc levels, but at normal doses it's very unlikely to push them into a harmful range. If you're taking a high-dose zinc supplement on top of this for a long stretch, just mention it to your pharmacist or doctor so they can keep an eye on things. For most people, a typical daily dose is nothing to worry about.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

This combination tablet is used in people with high blood pressure or congestive heart failure who develop low potassium (hypokalemia) when thiazides or similar diuretics are used alone.

It is also used when keeping potassium in the normal range is especially important, such as in people taking digitalis or those with significant heart rhythm problems. It can be used alone or added to other blood pressure medicines like methyldopa or beta blockers.
