1-Androsterone carries real health risks and hasn't been thoroughly tested for safety. The most common adverse effects reported are abnormal cholesterol levels (LDL cholesterol went up by around 29 mg/dL and HDL dropped by about 19 mg/dL in a 4-week study), kidney impairment (kidney function markers worsened and one measure of kidney health fell below normal), and liver enzyme elevations.

Because this is a hormone-altering compound, it should never be used during pregnancy or while breastfeeding due to unknown hormonal risks to your baby.
