Agaricus mushroom is generally well tolerated as a food, but supplement safety and long-term effects aren't well established. The most common side effects from taking it by mouth are abdominal discomfort, diarrhea, and nausea.

Rare serious effects include allergic reactions, liver damage (hepatotoxicity), and a lung condition called interstitial lung disease. Several cases of severe liver damage have been reported in women with cancer receiving chemotherapy who also took agaricus mushroom — two of those cases were fatal.

Allergic reactions including rash and lip swelling have also been reported. During pregnancy, there isn't enough safety data, so supplement doses should be avoided.

In one clinical study in patients with type 2 diabetes who are stabilized on conventional oral hypoglycemic agents, 3 of 29 patients taking an agaricus mushroom extract 500 mg three times daily for 12 weeks reported hypoglycemia, compared to one of 29 patients in the placebo group.
