D-Ribose is generally well tolerated for short-term use (up to about a month in studies), but long-term safety hasn't been well studied. The most common side effects when taken by mouth are diarrhea, gastrointestinal discomfort, headache, nausea, and low blood sugar (hypoglycemia).
A few people have reported lightheadedness, increased appetite, and mild to moderate anxiety — though lowering the dose resolved the anxiety and nausea in those cases. The safety data advises against using D-Ribose during pregnancy or while breastfeeding because there isn't enough information to know whether it's safe in those situations.
