FirstBiotic Advanced contains 15 active probiotic ingredients — beneficial live bacteria and yeasts meant to support gut health. The main strains are Lactobacillus acidophilus (two different forms), Bifidobacterium longum, Bifidobacterium bifidum, Lactobacillus delbrueckii, Bacillus coagulans, and seven other Lactobacillus and Bifidobacterium species.
The product also includes fructooligosaccharides (FOS), a type of fiber that may help feed beneficial bacteria. Beyond the active ingredients, the capsule contains several inert materials — glycerin, microcrystalline cellulose, hydroxypropyl methylcellulose, calcium laurate, chlorophyllin, silicon dioxide, vegetable cellulose, and water.
Does it work?
Moderate evidence
The evidence for most of these probiotics remains incomplete. Lactobacillus acidophilus (both strains) and Bifidobacterium bifidum are rated possibly effective for irritable bowel syndrome and possibly effective for antibiotic-associated diarrhea (L. acidophilus also for bacterial vaginosis and Helicobacter pylori infection, B. bifidum also for respiratory tract infections).
Lactobacillus delbrueckii, Bifidobacterium longum, and Bacillus coagulans have insufficient reliable evidence for most uses we checked. Fructooligosaccharides and Streptococcus thermophilus also show insufficient evidence across multiple conditions including constipation, diabetes, and cholesterol.
For many conditions on the label, we simply don't have enough reliable research to rate effectiveness.
How safe is it?
Well-documented data
These probiotics are generally well tolerated in healthy people. The most common side effects from the live organisms and FOS are mild gastrointestinal symptoms — bloating, flatulence, abdominal pain, and gas — especially when you first start or at higher doses.
People with compromised immune systems (including those on certain medications, after surgery, or with serious illness) should talk with their doctor before taking this product, as there are rare reports of infection from probiotic organisms in these patients. Limited safety data exist for supplement use during pregnancy and breastfeeding; Lactobacillus acidophilus and Bifidobacterium bifidum are rated possibly safe in pregnancy, but you should check with your doctor before taking any probiotic supplement while pregnant or nursing.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
