Glutamine is rated effective for sickle cell disease. For HIV/AIDS-related wasting, postoperative recovery, and critical illness from trauma, the evidence is possibly effective — meaning some studies support it, but more research is needed to be sure.

How safe is it?
Well-documented data
Orally, glutamine is generally well tolerated in healthy adults. Common side effects include belching, bloating, constipation, diarrhea, flatulence, stomach pain, headache, nausea, and vomiting — most often at high doses (10–30 grams daily).
Dizziness and headache have been reported; one person's dizziness resolved when the dose was lowered. Two people with bipolar disorder developed mania or hypomania on glutamine up to 4 grams daily.
People with kidney or liver disease should use glutamine only under medical supervision. There isn't enough safety data during pregnancy to know if supplement doses are okay — talk with your doctor or pharmacist for personalized advice.
The same goes for breastfeeding: there's little safety information, so avoid supplement doses unless a clinician recommends them.
