Horny goat weed is generally well tolerated in the short term, though human safety data is limited and quality varies between products. The most common side effects reported are dizziness, dry mouth, nosebleed, thirst, and vomiting.

Rarely, respiratory arrest has been noted. Long-term use may bring on vomiting, dry mouth, thirst, and nosebleed.

One case report described a 66-year-old man who developed an irregular heartbeat after two weeks of horny goat weed, though it's unclear if other ingredients played a role. Maca is generally well tolerated as a food but has not been studied long term as a supplement; raw maca can cause stomach pain.

Cowhage is generally well tolerated orally, though diarrhea, flatulence, and mucosal irritation are reported. Some clinical trials at higher doses (22.5 to 67.5 grams daily) noted nausea, vomiting, abdominal distention, insomnia in about 3% of participants, and headaches.
