The evidence for lion's mane mushroom on Alzheimer's disease, anxiety, athletic performance, cancer, cognitive function, and cognitive impairment is insufficient—the data we have doesn't support or rule out benefit for any of these uses. Kakadu plum is possibly effective for constipation, but the data for osteopenia, dyslipidemia (abnormal cholesterol or triglycerides), and hepatitis (liver inflammation) is also insufficient.
How safe is it?

Lion's mane mushroom is generally well tolerated as a food, but long-term supplement safety hasn't been well studied. The most common side effects are gastrointestinal discomfort, nausea, and skin rash.
Kakadu plum is generally well tolerated too, though it may cause flatulence and diarrhea; rarely, dried plums or plum pits have caused bowel obstruction. Since there isn't enough safety data in pregnancy or breastfeeding, both ingredients advise against use during those times—talk with your doctor or pharmacist about whether this product is right for you.

The most serious concern is a Moderate-severity interaction with blood thinners (anticoagulant/antiplatelet drugs). Both lion's mane extracts and kakadu plum may increase bleeding risk; the plum was studied in humans, where 200 mL of plum juice daily for 28 days prolonged clotting time and reduced how well platelets clump together, while lion's mane research is so far in vitro only.

Both lion's mane forms also interact Moderately with diabetes medications (antidiabetes drugs) and immunosuppressants. Animal research suggests lion's mane can lower blood sugar and raise insulin levels, potentially adding to the effect of your diabetes drug.
With immunosuppressants, lion's mane polysaccharides stimulate the immune system in lab and animal studies, which may work against the drug's job.
Additionally, kakadu plum's only other documented interaction is with blood thinners. No interactions are documented for kakadu plum with diabetes medications or immunosuppressants.

Lion's Mane Mycelium Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, lion's mane mushroom may increase the risk of bleeding when used with anticoagulant/antiplatelet drugs.
In vitro research suggests that lion's mane mushroom extracts can inhibit platelet aggregation.
Antidiabetes Drugs
Theoretically, lion's mane mushroom may have additive effects when used with antidiabetes drugs.
Animal research suggests that an aqueous extract of lion's mane mushroom can reduce serum glucose and increase serum insulin.
Immunosuppressants
Theoretically, concurrent use of lion's mane mushroom might interfere with immunosuppressive therapy.
In animal and in vitro research, lion's mane mushroom polysaccharides stimulate the immune system.
Kakadu Plum
Anticoagulant/Antiplatelet Drugs
Theoretically, plum juice might have antiplatelet effects.
Consuming plum juice while taking anticoagulant or antiplatelet drugs might increase the risk of bruising and bleeding. In healthy volunteers, drinking plum juice 200 mL daily for 28 days prolonged clotting time and inhibited platelet aggregation.
