Does it work?
Moderate evidence
Inositol is possibly effective for polycystic ovary syndrome (PCOS), metabolic syndrome, and preterm labor, though the evidence for anxiety, diabetic nerve pain, and depression is not strong. L-carnitine is effective for L-carnitine deficiency and possibly effective for high cholesterol, heart failure, and angina.
Choline has insufficient evidence in our data for athletic performance, age-related cognitive decline, and several other uses. Glucomannan is possibly effective for constipation, diabetes, and high cholesterol.
Chitosan is possibly effective for obesity, recovery after surgery, and blood pressure, though evidence for Crohn disease and dry eye is insufficient. We hold no effectiveness ratings for DL-methionine.
How safe is it?
Well-documented data
Inositol is generally well tolerated, though it can cause diarrhea, gas, and nausea at common doses. Safety in pregnancy isn't fully established — use only under medical supervision if you're pregnant.
L-carnitine is generally well tolerated at typical doses but can cause stomach upset and a fishy body odor at high doses; safety data in pregnancy and while breastfeeding are limited. Choline is rated likely safe in pregnancy when used as advised by your doctor, and your needs are actually higher while breastfeeding — but use supplements only under professional guidance.
High doses of choline (over 3.5 grams daily) can cause the same fishy odor, plus nausea, vomiting, and diarrhea. Glucomannan is generally well tolerated with plenty of water but can rarely cause choking or blockage if swallowed improperly; safety data in pregnancy and while breastfeeding are not sufficient to recommend it.
Chitosan is generally well tolerated short-term but should be avoided in pregnancy and while breastfeeding due to insufficient safety data — also watch for shellfish allergy since it's derived from shellfish.

Glucomannan
Oral Drugs
Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.
Inositol
Antidiabetes Drugs
Theoretically, taking inositol with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that inositol lowers blood glucose levels and glycated hemoglobin (HbA1c) levels in patients with diabetes.
L-Carnitine
Acenocoumarol (Sintrom)
Theoretically, L-carnitine might increase the anticoagulant effects of acenocoumarol.
L-carnitine might enhance the anticoagulant effects of acenocoumarol, an oral anticoagulant similar to warfarin, but shorter-acting. There are at least two case reports of INR elevation with concomitant use. In one case, a 33-year-old male with a previously stable INR had an elevated INR of 4.65 after L-carnitine was started and continued for 10 weeks. INR normalized after discontinuation of the L-carnitine-containing product.
Thyroid Hormone
Theoretically, L-carnitine might decrease the effectiveness of thyroid hormone replacement.
L-carnitine appears to act as a peripheral thyroid hormone antagonist by inhibiting entry of thyroid hormone into the nucleus of cells. Taking L-carnitine also seems to diminish some of the symptoms of hyperthyroidism.
Warfarin (Coumadin)
Theoretically, L-carnitine might increase the anticoagulant effects of warfarin.
L-carnitine might increase the anticoagulant effects of acenocoumarol, a shorter-acting oral anticoagulant similar to warfarin. There is not enough information to know whether this interaction occurs with L-carnitine and warfarin.
Choline Bitartrate
Atropine
Theoretically, choline might decrease the effects of atropine in the brain.
Animal research shows that administering choline one hour before administering atropine can attenuate atropine-induced decreases in brain levels of acetylcholine. Theoretically, concomitant use of choline and atropine may decrease the effects of atropine.

Acyclovir (Zovirax)
Chitosan can reduce the absorption of acyclovir, potentially increasing the risk for treatment failure.
Clinical research in humans shows that taking chitosan along with acyclovir 200 mg reduces acyclovir absorption. Concomitant administration of chitosan 400 mg or 1000 mg reduced the acyclovir area under the curve (AUC) and peak plasma concentration by about 30% and 40%, respectively, compared with control. Concomitant administration with chitosan 1000 mg also increased time to peak concentration from 1 hour to 2 hours. In vitro research suggests that the mechanism for reduced absorption is due to acyclovir entrapment in chitosan-mucus complexes, which reduces intestinal absorption.
Warfarin (Coumadin)
Theoretically, chitosan might increase the risk of bleeding when taken with warfarin.
In a case report, a patient taking warfarin had a significantly increased international normalized ratio (INR) after starting chitosan 1200 mg daily. The INR normalized after chitosan was discontinued and vitamin K was administered. The patient once again started taking chitosan and again had a significant increase in INR. The INR stabilized again once chitosan was discontinued. Researchers theorize that this interaction might occur because chitosan decreases absorption of fat-soluble vitamins, including vitamin K, which could increase the anticoagulant effect of warfarin.
