MacuGuard has 6 active ingredients designed to support eye health, mainly carotenoids (pigments that accumulate in the macula of the eye). You get zeaxanthin in two forms—meso-zeaxanthin and trans-zeaxanthin—plus free lutein, natural astaxanthin, and extracts from marigold and a proprietary carotenoid-phospholipid blend.
The product also contains inactive ingredients like olive oil, gelatin, safflower oil, and beeswax to form the softgel capsule.
Does it work?
Moderate evidence
For age-related macular degeneration (AMD), the main reason people take this supplement, both zeaxanthin and lutein are rated possibly effective. The evidence suggests they may help, but it's not definitive.
For other uses listed in the data—cognitive decline, Alzheimer disease, asthma, cancer, and several others—the evidence we hold is insufficient to rate whether these ingredients work. Astaxanthin's effectiveness for any of its studied uses is also not established in our data.
How safe is it?
Well-documented data
Zeaxanthin and lutein from the supplement are generally well tolerated at typical doses. Lutein at up to 20 mg daily hasn't caused adverse effects in the studies we hold.
Astaxanthin is also generally well tolerated in short-term studies, though long-term safety data are limited; the main side effects reported are abdominal pain, diarrhea, and red-colored stools at higher doses (40 mg daily). For pregnancy and breastfeeding: zeaxanthin and lutein are rated likely safe, but the data advise caution with supplements during pregnancy and breastfeeding since concentrated supplement safety hasn't been well studied—dietary amounts from food are fine.
Astaxanthin is different: the safety data advise against use during both pregnancy and breastfeeding due to lack of evidence. We hold no pregnancy/breastfeeding data for phospholipids or C3G.

MacuGuard by Life Extension contains two carotenoid ingredients with documented interactions: zeaxanthin (in both meso- and trans- forms) and astaxanthin.
The most serious concern is astaxanthin's effect on liver enzymes — it may speed up how your body breaks down drugs metabolized by CYP3A4 and CYP2B6 enzymes, potentially lowering their levels and making them less effective. This is a Moderate-severity theoretical risk so far observed only in lab studies, not yet reported in people.

Zeaxanthin (the two forms together) carries a Moderate interaction with antidiabetes drugs. In animal models, zeaxanthin has lowered blood sugar, and theoretically combining it with diabetes medications could raise the risk of low blood sugar (hypoglycemia).
However, population studies haven't found that eating more zeaxanthin and lutein prevents type 2 diabetes, so the real-world significance remains unclear.

natural Astaxanthin
Cytochrome P450 2B6 (Cyp2B6) Substrates
Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP2B6.
In vitro research shows that astaxanthin induces cytochrome CYP2B6 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.
Marigold extract
Cns Depressants
Theoretically, calendula might have additive effects when used with CNS depressants, although this appears to be unlikely.
Although some animal research has suggested that a saponoside constituent in calendula may have sedative effects, calendula has been used for over 30 years without reports of sedation in humans.
