Boswellia serrata is generally well tolerated short-term when taken by mouth. The most common side effects are mild: abdominal pain, diarrhea, headache, heartburn, itching, and nausea.

Some people also report dizziness or vertigo; in one study, about 11% reported headache. Rarely, large amounts can cause a bezoar (a mass in the stomach).

Do not use this product during pregnancy — there isn't enough safety data. Avoid it while breastfeeding as well, since safety is unknown.

However, other in vitro research suggests that, when coupled with calcium ions, boswellic acids containing the keto group have immunostimulant properties within specific cell signaling pathways.
