CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Acetaminophen, Caffeine, Codeine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with this particular combo: codeine is a prodrug. It doesn't do much on its own. Your body has to convert it into morphine using an enzyme called CYP2D6 before you feel real pain relief. Lab studies hint that Boswellia may slow that same enzyme down a bit, which could mean less codeine gets turned into morphine, and possibly weaker pain control rather than more.
I want to be honest, though: this is theory only, based on test-tube research, not real patients. So I wouldn't lose sleep over it. The acetaminophen and caffeine pieces aren't really affected here. If you take this regularly for pain, just mention the Boswellia to your pharmacist or doctor so they can keep an eye on whether it's still working for you.

This combo pill has three parts, and the one most likely to feel the effect of Boswellia is the caffeine. Your body clears caffeine using an enzyme called CYP1A2, and lab studies hint that boswellic acids may slow that enzyme down. If caffeine clears more slowly, it could hang around longer, so some folks might notice extra jitteriness, a racing heart, or trouble sleeping.
Here's the honest part: this is only a theoretical, test-tube finding, not something proven in real people, so for most it likely won't be a big deal. The codeine and acetaminophen pieces aren't really driven by this pathway. Still, if you take this medicine regularly and want to add Boswellia, run it by your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill carries codeine, which is actually a prodrug. Your body has to convert it into morphine before it relieves pain, mostly using an enzyme called CYP2D6. The interaction on file involves a different enzyme, CYP3A4, which plays a smaller, secondary role in handling codeine and helps break down the caffeine too. So the main effect here isn't huge for the codeine's pain relief.
The whole concern is theoretical, based only on lab-dish studies where Boswellia mostly slowed CYP3A4 (and sometimes sped it up slightly). In real life this rarely amounts to much, but if you ever feel extra drowsy, foggy, or notice more jitteriness from the caffeine, mention it to your pharmacist or doctor before changing anything.
