Acetaminophen, Caffeine, Codeine, Salicylamide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product contains codeine, and that is the piece worth focusing on here. Your body relies on an enzyme called CYP2D6 to turn codeine into morphine, which is what actually relieves pain. Lab studies hint that boswellic acids may slow down that same enzyme. Because codeine is a prodrug that needs CYP2D6 to become active, blocking the enzyme would likely mean less pain relief, not more.
The good news: this is only a theoretical concern based on test-tube data, not real patients, so the practical risk is probably small. The caffeine, acetaminophen, and salicylamide parts aren't really affected this way. Still, if your pain isn't well controlled, mention the boswellia to your pharmacist or doctor.

Here's the piece that matters with this combination pill. Of its ingredients, the caffeine is the one cleared mainly by an enzyme called CYP1A2. In lab studies (not in people yet), boswellic acids appear to slow that enzyme down. If that happened in your body, caffeine could hang around longer than usual, so you might notice jitteriness, a racing heart, trouble sleeping, or feeling wired, especially if you also drink coffee or energy drinks.
I want to be honest about the strength here: this is theoretical, based only on test-tube data, so it may not amount to much in real life. The codeine and acetaminophen parts work through different pathways and aren't really the concern. Still, if you notice extra caffeine-type symptoms, mention it to your pharmacist or doctor before changing anything.

In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about this combo pill: the codeine part is what matters most when we talk about Boswellia. Codeine is a prodrug, meaning your body has to convert it into morphine to give pain relief. That activation mostly runs through a different enzyme (CYP2D6), but CYP3A4 also handles part of codeine's processing. Since lab studies hint that Boswellia might lightly block CYP3A4, the real-world effect on this medicine is hard to predict and probably small.
I want to be honest that this is theoretical, based only on test-tube research, not real patients. If anything shifted, you might notice slightly more drowsiness, constipation, or nausea. Just run it by your pharmacist or doctor before combining them, especially since codeine affects everyone a bit differently.
