The evidence behind L-glutamine is strongest for sickle cell disease, where it's rated effective. For HIV/AIDS-related wasting, postoperative recovery, and critical illness from trauma, the evidence is rated possibly effective — meaning there's promise but the data isn't yet conclusive.

How safe is it?
Well-documented data
L-glutamine is generally well tolerated in healthy adults. The most common side effects are gastrointestinal: belching, bloating, constipation, diarrhea, flatulence, nausea, and vomiting — especially at higher doses.
Headache and musculoskeletal pain have also been reported. Two people with bipolar disorder developed mania or hypomania at doses up to 4 grams daily, though this is rare.
People with kidney or liver disease should take this only under a doctor's supervision. There isn't enough safety data to recommend supplemental doses during pregnancy without talking to your doctor first, and breastfeeding safety data is limited — discuss it with your clinician if you're nursing.
