Does it work?
Moderate evidence
For Lactobacillus acidophilus DDS-1, the evidence is rated possibly effective for Helicobacter pylori, bacterial vaginosis, antibiotic-associated diarrhea, and irritable bowel syndrome (IBS). It is rated possibly ineffective for atopic disease and has insufficient reliable evidence to rate for allergic rhinitis (hay fever).
For Bifidobacterium bifidum UABb-10, the evidence is rated possibly effective for IBS and respiratory tract infections. It has insufficient reliable evidence to rate for allergic rhinitis, acute respiratory distress syndrome (ARDS), and age-related cognitive decline.

How safe is it?
Well-documented data
Lactobacillus acidophilus is described as generally well tolerated in healthy adults and children, though the data also notes it should be checked with your doctor before use. Gastrointestinal side effects such as bloating, gas, and diarrhea are described as uncommon.
Rare serious infections, such as bloodstream infections, have been isolated in some patients, mostly when bacteria move from their usual location in the body. Bifidobacterium bifidum is generally well tolerated in healthy people, but those with weakened immune systems should be cautious.
Bloating and gas have been reported with probiotic use in general, not specifically with B. bifidum. Bifidobacterium sepsis has occurred in critically ill patients, but these cases are rare and none seem to be due to B. bifidum.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
