Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Ultra Formula Co Q10 400 mg contains one active ingredient: coenzyme Q10 at 400 mg per capsule. Coenzyme Q10 (also called ubiquinone) is a compound your body produces naturally and uses in cells to generate energy and protect against oxidative damage.
The capsule also contains inactive ingredients — vegetable cellulose, rice bran, oat fiber, and magnesium stearate — which help form and stabilize the capsule.
Does it work?
Strong evidence
Coenzyme Q10 is likely effective for people with coenzyme Q10 deficiency. For other uses, the evidence is more limited: it's possibly effective for fibromyalgia, migraine headache, congestive heart failure, and diabetic neuropathy, though the research is still developing.
If you're taking this to address a specific health concern, talk with your doctor or pharmacist about whether the evidence supports its use for your situation.
How safe is it?
Well-documented data
Coenzyme Q10 is generally well tolerated when taken as directed. The most common side effects are gastrointestinal — appetite suppression, diarrhea, heartburn, nausea, and vomiting — and these occur in less than 1% of people; splitting larger daily doses may help minimize stomach upset.
Headache, dizziness, and insomnia have been reported. Skin itching and allergic rash are rare.
There isn't enough safety data during pregnancy to say whether it's safe, so it's best to avoid it unless your doctor advises otherwise. Safety data for breastfeeding is limited; talk with your doctor before using this product while nursing.
Meds to double-check
Moderate interaction found
Before taking this product, check with your doctor or pharmacist if you take warfarin (Coumadin) or other blood thinners, chemotherapy drugs (especially alkylating agents like cyclophosphamide), or medications for high blood pressure. Coenzyme Q10 may reduce how well these medications work or could increase their effect on your blood pressure.
The search tool on this page can help you check your specific drugs.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Coenzyme Q10 400 mg is generally well tolerated and may help with coenzyme Q10 deficiency or possibly support heart health, migraine control, or diabetic nerve pain — though evidence varies. If you take warfarin, chemotherapy, or blood pressure medication, you need to check with your doctor or pharmacist before starting, because coenzyme Q10 can interfere with how these drugs work.
Same goes for pregnancy and breastfeeding — discuss it with your healthcare provider first.

Ultra Formula Co Q10 400 mg by 21st Century Drug Interactions
HelloPharmacist Interaction Report
Ultra Formula Co Q10 400 mg by 21st Century contains coenzyme Q10, which has documented interactions with several medication types.
The most serious concern is with chemotherapy drugs called alkylating agents (such as cyclophosphamide) — coenzyme Q10's antioxidant effects may theoretically reduce how well these cancer medications work by protecting tumor cells from the oxidative stress these drugs rely on to kill cancer cells.
Read the full breakdown — every affected drug type, severity by severity
Coenzyme Q10 also interacts with warfarin (Coumadin), a blood thinner. Because coenzyme Q10 is chemically similar to vitamin K, it may have blood-clotting effects that could weaken warfarin's ability to prevent clots; a handful of cases have been reported where people's warfarin stopped working as well after starting coenzyme Q10.
Lastly, coenzyme Q10 may have additive blood pressure-lowering effects when combined with antihypertensive drugs used to manage high blood pressure, though the strength of this effect varies from person to person.
Altogether, these interactions span 198 individual medications. Use the search tool on this page to check your exact prescriptions before starting.

For each ingredient in Ultra Formula Co Q10 400 mg with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
Coenzyme Q10
Alkylating Agents
Coenzyme Q10 has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals.
Theoretically, antioxidants such as coenzyme Q10 might protect tumor cells from chemotherapeutic agents that work by inducing oxidative stress, such as alkylating agents (e.g., cyclophosphamide) and radiation therapy. The clinical importance of this interaction is unknown.
Warfarin (Coumadin)
Coenzyme Q10 is chemically similar to menaquinone and might have vitamin K-like procoagulant effects, which could decrease the effects of warfarin.
Concomitant use of coenzyme Q10 and warfarin might reduce the anticoagulant effects of warfarin. Four cases of decreased warfarin efficacy thought to be due to coenzyme Q10 have been reported. However, there is some preliminary clinical research that suggests coenzyme Q10 might not significantly decrease the effects of warfarin in patients who have a stable INR.
Antihypertensive Drugs
Theoretically, coenzyme Q10 might have additive effects with antihypertensive drugs.
Some clinical research shows that coenzyme Q10 can significantly lower blood pressure, although other studies have shown conflicting results.
