Why this applies to Valsartan

Valsartan falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group St. John's Wort interacts with.

St. John's wort may increase the metabolism and reduce the levels of CYP2C9 substrates.
There is contradictory research about the effect of St. John's wort on CYP2C9. Some in vitro research shows that St. John's wort induces CYP2C9, but to a lesser extent than CYP3A4. St. John's wort also induces metabolism of the S-warfarin isomer, which is a CYP2C9 substrate. Other research shows that St. John's wort 300 mg three times daily for 21 days does not significantly affect the pharmacokinetics of a single 400 mg dose of ibuprofen, which is also a CYP2C9 substrate. Until more is known, use St. John's wort cautiously in patients who are taking CYP2C9 substrates.

Here's the basic idea with valsartan (Diovan) and St. John's wort. The herb can speed up certain liver enzymes that break down medicines. In theory, that could clear valsartan from your body a little faster, which might make it work less well. If that happened, you could see your blood pressure creep back up, or your heart failure symptoms feel less controlled.

That said, I'd put this in the possible, not proven bucket. Valsartan is only partly handled by this enzyme pathway, and the research on St. John's wort and these enzymes is genuinely mixed. So this is more of a caution than a red alert. Still, since blood pressure control matters, please loop in your pharmacist or doctor before combining the two, and keep an eye on your home readings if you do.

About Valsartan

What Is Valsartan Used For?

Brand names: Diovan, Prexxartan

Valsartan is used to treat high blood pressure (hypertension) in adults and children, heart failure in adults, and to lower the risk of cardiovascular death in adults who have had a heart attack and have reduced heart function. Lowering blood pressure reduces the risk of stroke and heart attack.

Diovan (valsartan) tablets are approved to treat high blood pressure in children as young as 1 year old. Valsartan tablets (generic) are approved for children 6 years and older. Valsartan oral solution is specifically indicated for patients with heart failure or post-heart attack left ventricular dysfunction who are unable to swallow tablets.
