Here's the gist with agmatine and insulin: insulin already lowers your blood sugar, and there's a thought that agmatine might nudge it down a little further. So the worry is stacking two blood-sugar-lowering effects and dipping too low (hypoglycemia). That can show up as shakiness, sweating, a fast heartbeat, confusion, or feeling lightheaded.

I want to be honest about how shaky this evidence is, though. The sugar-lowering effect of agmatine has mostly been seen in lab dishes and animal studies, not solid human trials, so it's theoretical. If you do try agmatine, it's reasonable to check your blood sugar a bit more often at first and loop in your pharmacist or doctor before changing anything with your insulin.
