Amiloride, Hydrochlorothiazide falls under DIURETIC DRUGS — the group Alder Buckthorn interacts with.

Alder buckthorn has stimulant laxative effects. Theoretically, overuse of alder buckthorn might compound diuretic-induced potassium loss. There is some concern that people taking alder buckthorn along with potassium depleting diuretics might have an increased risk for hypokalemia.
Some diuretics that can deplete potassium include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), and hydrochlorothiazide (HCTZ, HydroDIURIL, Microzide), and others.

Here's the thing about your water pill: it's actually a combo. The amiloride half is a potassium-sparing diuretic, so it helps your body hang onto potassium, while the hydrochlorothiazide half tends to push potassium out. The makers paired them on purpose to balance those effects. Alder buckthorn, used as a stimulant laxative, can also drain potassium when overused (or used too long), mostly through fluid and electrolyte loss in the bowel.

So in theory, leaning on alder buckthorn could nudge your potassium lower and partly undo that built-in balance, possibly leaving you with muscle cramps, weakness, or an irregular heartbeat. Honestly, this concern is based on pharmacology, not strong studies, and brief, occasional use is less worrisome than daily use. Still, please run it by your pharmacist or doctor first.

What Is Amiloride and Hydrochlorothiazide Used For?

Brand names: Moduretic, Amilzide, Moduret 25, Amil-Co

Amiloride hydrochloride and hydrochlorothiazide tablets are used for patients with high blood pressure (hypertension) or congestive heart failure who tend to develop low potassium (hypokalemia) when taking a thiazide or other diuretic alone — or for whom keeping potassium levels in a normal range is especially important, such as people taking digoxin or those with significant heart rhythm problems.

The tablets may be used on their own or alongside other blood pressure medicines, such as methyldopa or beta blockers. This combination product is not intended as the first-choice treatment for swelling (edema) or high blood pressure unless low potassium is a specific concern.
