Theoretically, alfalfa might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Animal research suggests that alfalfa decreases blood sugar in diabetic mice. Also, in one case report, a diabetic patient experienced hypoglycemia after consuming alfalfa extract. Monitor blood glucose levels closely. Dose adjustments might be necessary.

Here's the gist: alfalfa may have a mild blood sugar lowering effect of its own. If you're injecting pork insulin, which already pulls your glucose down, stacking alfalfa on top could nudge your sugar a little lower than expected. That can mean hypoglycemia, with signs like shakiness, sweating, a racing heart, confusion, or feeling lightheaded.

Now, the honest part: this is mostly theoretical. It comes from diabetic mice and a single patient case report, not strong human studies, and small culinary amounts of alfalfa sprouts are unlikely to do much. Concentrated alfalfa extracts are the bigger question mark. If you want to try it, just check your glucose more often at first and talk with your doctor or pharmacist before adding it, since your insulin dose might need a small tweak.
