Here's the gist with alfalfa and pomalidomide (Pomalyst). Pomalidomide isn't a typical immune-suppressing drug, but it does work partly by adjusting your immune system to fight myeloma or Kaposi's sarcoma. Alfalfa may have a mild immune-stimulating effect (it contains a compound called L-canavanine plus phytoestrogens), so the worry is that, in theory, it could tug your immune system in a different direction and possibly blunt how well your cancer treatment works.

I want to be honest: this concern comes from lab studies and a few case reports, not from solid human trials on this exact drug, so it's theoretical. Still, when you're on serious cancer therapy, I'd rather play it safe. Please run any alfalfa supplement past your oncology team or pharmacist before starting, and don't change your pomalidomide on your own.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
