Why this applies to Maraviroc

Maraviroc falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the short version: your body clears maraviroc mostly through a liver enzyme called CYP3A4. In animal studies, high doses of atractylodes appeared to slow down a similar enzyme. If that same slowing happened in people, your maraviroc could clear a bit more slowly and build up, which might mean more of its usual side effects like dizziness, lightheadedness when standing, headache, or stomach upset.

I want to be honest about how thin this evidence is. This is only a theoretical concern from lab and animal work, and it hasn't been shown in humans. So I'm not raising any alarms. Since maraviroc is part of your HIV treatment, please don't change anything on your own. Just give your pharmacist or doctor a quick heads-up before adding atractylodes so they can keep an eye on things.

Atractylodes is a root used for centuries in traditional Chinese, Japanese, and Thai medicine, mostly for digestive complaints and fatigue, often as part of multi-herb formulas. Modern research is still early and mostly from lab and animal studies, so its true benefits in people are not well proven. If you want to try it, talk with a pharmacist or doctor first, and avoid it during pregnancy and breastfeeding.

About Maraviroc

What Is Maraviroc Used For?

Brand names: Celsentri, Selzentry

Maraviroc treats a specific type of HIV-1 infection called CCR5-tropic HIV-1. It is always used alongside other antiretroviral (anti-HIV) medicines — never alone. A special test called a tropism assay must be done before starting treatment to confirm you have the right type of HIV.

Selzentry (the brand-name oral solution and tablet) is approved for adults and pediatric patients weighing at least 2 kg. Generic maraviroc tablets are approved for adults and pediatric patients aged 2 years and older weighing at least 10 kg. Neither form is recommended for patients whose HIV is classified as dual/mixed-tropic or CXCR4-tropic.
