Nirogacestat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the gist for anyone taking Ogsiveo (nirogacestat) for desmoid tumors alongside atractylodes. Nirogacestat is an active drug (not a prodrug) that your body clears mainly through a liver enzyme called CYP3A4. In animal studies, high doses of atractylodes appeared to slow down a similar enzyme. If that same slowdown happened in people, nirogacestat could build up a bit and its side effects might feel stronger, things like diarrhea, nausea, mouth sores, rash, or fatigue.

I want to be honest, though: this is only a theoretical concern from lab and animal work, and it has never been shown in humans. Still, because nirogacestat is a serious cancer medicine with real interaction warnings, please run atractylodes by your oncology team or pharmacist before adding it.

Ogsiveo (nirogacestat) is approved for adults with desmoid tumors that are progressing and require systemic (whole-body) treatment. Desmoid tumors are rare, locally aggressive growths of connective tissue.

This indication is specific to Ogsiveo tablets taken by mouth. It is not approved for children, as safety and effectiveness in pediatric patients have not been established.
