Pimavanserin Tartrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the simple version. Your body uses an enzyme called CYP3A4 to clear pimavanserin (Nuplazid) out of your system. In animal studies using very high doses, atractylodes seemed to slow down a similar enzyme. If that were to happen in people, pimavanserin could build up a bit higher than usual, which in theory might mean more of its side effects, things like swelling in the legs, nausea, constipation, confusion, or an irregular heartbeat.

The key word here is theory. This effect has only shown up in lab and animal work, never in humans, so it's rated as a minor, possible concern. Still, since pimavanserin can affect heart rhythm, I'd mention any atractylodes-containing product to your pharmacist or doctor before combining them.

What Is Pimavanserin Used For?

Brand name: Nuplazid

Pimavanserin — sold as Nuplazid and also available as a generic capsule — is approved to treat hallucinations and delusions in adults with Parkinson's disease psychosis. These are the unsettling false perceptions and beliefs that can occur as part of Parkinson's disease.

It is not approved for psychosis caused by other forms of dementia. Using it in elderly patients with non-Parkinson's dementia-related psychosis carries serious safety risks.
