Why this applies to Polatuzumab Vedotin-Piiq
Polatuzumab Vedotin-Piiq falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Atractylodes interacts with.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Polivy (polatuzumab vedotin-piiq) and atractylodes. Polivy is an antibody-drug conjugate that releases a cell-killing payload called MMAE, and that MMAE portion is partly broken down by the liver enzyme CYP3A4. The thought here is that atractylodes might slow down CYP3A4, which could let a bit more of the active drug linger in your system.
If that happened, you could in theory see more of Polivy's usual side effects, like nerve tingling or numbness in the hands and feet, low blood counts, fatigue, or infections. That said, this is a theoretical concern. It comes only from animal data using high doses, and it has not been shown in people. Still, since this is a serious cancer treatment, please run any supplement past your oncology team or pharmacist before starting it.

Polatuzumab vedotin-piiq injection is used along with bendamustine (Belrapzo, Treanda) and rituximab (Rituxan) in adults to treat a certain type of non-Hodgkin's lymphoma (NHL; a type of cancer that begins in a type of white blood cells that normally fights infection) that did not improve or improved but returned after treatment with at least two other chemotherapy medications. Polatuzumab vedotin-piiq is in a class of medications called antibody-drug conjugates. It works by killing cancer cells.
