Theoretically, beet might increase the levels of CYP3A4 substrates.
In vitro research suggests that betanin, the major pigment in beet, competitively inhibits CYP3A4 in a dose-dependent manner similarly to strong CYP3A4 inhibitor ketoconazole.

Here's something interesting about losartan: it's actually a bit of a two-step drug. Your liver uses an enzyme called CYP3A4 (along with another one, CYP2C9) to turn losartan into its more powerful active form, which is the part that does most of the work lowering your blood pressure. The lab note we have suggests a pigment in beets, betanin, might slow CYP3A4 down.

So in theory, beets could blunt how much active drug you make, possibly making losartan a touch less effective. But honestly, this is based only on test-tube research, not real people, so I wouldn't lose sleep over it. Eating beets at dinner is very different from taking concentrated supplements. If you want to use beetroot regularly, just give your pharmacist or doctor a heads-up so they can keep an eye on your blood pressure.

Beet and beetroot contain nitrates, which can cause vasodilation, potentially leading to lower blood pressure. However, a study published in the European Journal of Clinical Nutrition using concentrated beetroot juice found no significant impact on blood pressure or heart rate in different age groups. Other small clinical studies suggest that while beet consumption might transiently lower blood pressure due to vessel dilation, there's no consistent evidence of a lasting effect. Overall, the theoretical risk of reduced blood pressure due to beet's nitrate content exists, but studies generally indicate a low and temporary impact rather than a sustained decrease.

Here's the simple version when it comes to beets and your losartan (Cozaar). Both can nudge your blood pressure in the same direction, downward. Losartan blocks a hormone called angiotensin II so your vessels relax, and the natural nitrates in beets can also help widen blood vessels a little. Stacked together, they could, in theory, lower your numbers a touch more than losartan alone.

The good news is that the research here is reassuring. Studies, including ones using concentrated beetroot juice, found any effect tends to be small and short-lived, not a big lasting drop. Food-sized servings of beets are very unlikely to cause a problem. If you're using strong beetroot supplements daily, just mention it to your pharmacist and keep an eye out for lightheadedness or dizziness. Don't change your losartan on your own.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
