Alfentanil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.

Theoretically, BMPEA might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that BMPEA inhibits CYP3A4 by up to 37%.

Here's the gist for these two together. Alfentanil is a powerful opioid pain reliever, usually given during procedures or anesthesia, and your body clears it through a liver enzyme called CYP3A4. In lab studies, BMPEA appears to slow that same enzyme down by about a third. If that holds true in people, alfentanil could hang around longer and hit harder, which means more drowsiness, slowed breathing, low blood pressure, and deep sedation. With a strong opioid, that's nothing to take lightly.

I'll be honest though, this is theoretical right now, based only on test-tube data, not real patients. On top of that, BMPEA is an unapproved stimulant best avoided anyway. Please loop in your doctor or pharmacist before mixing the two.
