Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.

Theoretically, BMPEA might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that BMPEA inhibits CYP3A4 by up to 37%.

Here's the basic idea with these two together. Your body relies on an enzyme called CYP3A4 to break down and clear larotrectinib (Vitrakvi). BMPEA appears to slow that enzyme down a bit, at least in lab dishes, where it blocked CYP3A4 by about a third. If that holds true in people, larotrectinib could clear more slowly and build up in your system, which may mean stronger effects and a higher chance of side effects like dizziness, tiredness, nausea, or changes in liver labs.

I'll be honest with you though, this one is theoretical and based only on test-tube data, so we don't really know how much it matters in real life. On top of that, BMPEA is a risky, unapproved stimulant I'd steer clear of anyway. Please loop in your oncology team or pharmacist before mixing anything new with Vitrakvi.

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration that can drive tumor growth. It is used when the tumor is metastatic (has spread) or when surgery to remove it would likely cause serious harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients must be selected for Vitrakvi therapy based on an FDA-approved test that confirms the NTRK gene fusion in their tumor. This approval is under the FDA's accelerated approval pathway, based on how many patients responded to treatment and how long those responses lasted. Continued approval may depend on results from follow-up clinical trials.
