Mitapivat falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Beta-methylphenethylamine (bmpea) interacts with.

Theoretically, BMPEA might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that BMPEA inhibits CYP3A4 by up to 37%.

Here's the basic idea with these two together. Your body relies on an enzyme called CYP3A4 to help break down and clear mitapivat (Pyrukynd). Lab studies suggest BMPEA can partly block that enzyme (by around a third in test-tube research). If that happens in real life, mitapivat could build up a bit higher than usual, which might mean stronger effects and a greater chance of side effects like headache, nausea, back or joint pain, or mood changes.

That said, this is theoretical, based only on early lab data, not on real patients. So I don't want to alarm you. The bigger concern is BMPEA itself, since it's an unapproved amphetamine-like stimulant with real heart risks. I'd skip it and check in with your doctor or pharmacist before mixing it with mitapivat.

Beta-methylphenethylamine (BMPEA) is a synthetic stimulant chemically similar to amphetamine that has been found in some weight-loss and sports supplements, often without being clearly listed on the label. What Is Mitapivat Used For?

Brand name: Pyrukynd

Mitapivat is available as two separate branded products approved for different conditions. Pyrukynd is used to treat hemolytic anemia — a type of anemia caused by red blood cells breaking down too quickly — in adults who have pyruvate kinase (PK) deficiency, a rare inherited blood disorder. Aqvesme is used to treat anemia in adults with alpha- or beta-thalassemia, inherited conditions that affect how the body makes hemoglobin.

These are distinct approvals. Pyrukynd and Aqvesme are not interchangeable, and each is approved only for its specific indication.
