Why this applies to Probenecid

Probenecid falls under ORAL DRUGS — the group Black Psyllium interacts with.

Theoretically, psyllium might increase, decrease, or have no effect on the absorption of oral drugs.
Psyllium seems to have variable effects on drug absorption. To avoid changes in absorption, take psyllium 30-60 minutes after oral medications. Animal research shows that blond psyllium delays and increases the absorption of metformin and ethinyl estradiol. Case reports and animal research suggest that blond psyllium might reduce absorption of lithium, digoxin, olanzapine, and carbamazepine. Finally, some pharmacokinetic studies show that psyllium does not affect the absorption of levothyroxine or warfarin. Although many of these studies evaluated blond psyllium, the fiber content in black psyllium may have similar effects.

Here's the simple version: black psyllium is a soluble fiber that turns into a thick gel in your gut. That gel can grab onto pills sitting in your digestive tract and slow down or reduce how much gets absorbed. With probenecid, the worry is that less of your dose might make it into your bloodstream. Probenecid is not a prodrug, so weaker absorption simply means a weaker effect, which for gout could mean your uric acid isn't controlled as well over time.

The good news is this is rated minor and mostly theoretical, and it's easy to sidestep. Just take your probenecid first, then wait about 30 to 60 minutes before your psyllium. If you have questions about timing, your pharmacist can help.

Black psyllium is a soluble-fiber supplement made from the seeds of a Plantago plant, used mostly to ease constipation and support digestive health. It is best known and most studied for relieving constipation and may modestly help lower cholesterol, but it must always be taken with plenty of water. What Is Probenecid Used For?

Brand names: Benemid, Benuryl, Probecid

Probenecid tablets are used to treat hyperuricemia (high uric acid levels in the blood) associated with gout and gouty arthritis. By helping the kidneys remove more uric acid, it lowers uric acid levels over time and helps prevent painful gout attacks.

Probenecid tablets are also used alongside certain penicillin-type antibiotics — including penicillin, ampicillin, methicillin, oxacillin, cloxacillin, and nafcillin — to raise and sustain the antibiotic's level in the bloodstream, making the antibiotic work more effectively.
